DOT Annual Inspection Checklist: What Inspectors Actually Look For

BINTEK Dispatch Team · Sep 4, 2026

The annual inspection isn't a rubber stamp. Here's the real checklist we run on commercial units — brakes, lights, tires, suspension, and the paperwork that fails the most trucks.

A DOT annual (periodic) inspection is a full-vehicle safety evaluation, not a quick walkaround. The inspector is checking that the truck is legal to operate on public roads. The trucks that fail aren't usually broken in an obvious way — they fail on small, deferred items that compound.

Brakes

  • ▸Pushrod stroke at full-application pressure — out-of-adjustment brakes are the top failure.
  • ▸Lining thickness on all foundation brakes; no pad-to-rotor contact.
  • ▸Air system build rate and leaks — the system must hold pressure with the engine off.
  • ▸ABS function and no stored active fault codes.

Lights & reflectors

  • ▸Every required lamp operable: headlamps, taillights, brake, turn, clearance, identification, and plate lights.
  • ▸Lenses intact — cracked or taped lenses fail even if the bulb works.
  • ▸Reflective tape present and in fair condition on trailers.

Tires & wheels

  • ▸Minimum tread depth (4/32 steer, 2/32 elsewhere) and no flatspots, repairs in the crown, or exposed cord.
  • ▸Wheel torque verified — loose lug nuts are a roadside-out-of-service item.
  • ▸Inflation pressure checked hot and cold; no mixed-matched duals.

Suspension & steering

  • ▸No broken leaves, collapsed air bags, or loose U-bolts.
  • ▸Steering linkage free of excessive play; no looseness at the kingpin or fifth wheel.

The paperwork that fails the most trucks

The inspection report itself is the most common failure. Missing signatures, wrong mileage, no defect notation, or an expired annual decal will get a truck cited faster than a worn pad. BINTEK provides a completed, signable inspection report for every annual we run, and we keep the record on file for your audit trail.
